Photo by Nathalie Walker<\/p><\/div>\n<p style=\"text-align: center;\"><strong>Celebrating 50 years, JABSOM has made itself known as a leading medical school in the nation. And with an Ebola\u00a0<\/strong><strong>vaccine currently in development at its Kakaako campus, JABSOM is on the path to making a difference in medicine<\/strong><\/p>\n<p class=\"p1\"><span class=\"s1\">I<\/span>t was Dec. 26, 2013, in\u00a0Meliandou, Guinea,\u00a0when an 18-month-old\u00a0boy developed a fever. Two\u00a0days later, Ebola had taken his\u00a0life.<\/p>\n<p class=\"p1\">This, says World Health\u00a0Organization, is where it all\u00a0began. In the weeks that followed,\u00a0the illness spread rapidly\u00a0to become what the Centers for\u00a0Disease Control and Prevention\u00a0has classified as the largest\u00a0Ebola outbreak in history. The\u00a0media attention it received and\u00a0subsequent frenzy after Ebola\u00a0made its way into the U.S.\u00a0made it feel new \u2014 surreal,\u00a0almost.<\/p>\n<p class=\"p1\">But the reality is that Ebola\u00a0already had existed for almost\u00a040 years in Africa, having first\u00a0been discovered in 1976. And\u00a0here in Hawaii, halfway around\u00a0the world, development of a\u00a0vaccine for the deadly disease\u00a0was well underway at John A.\u00a0Burns School of Medicine\u00a0(JABSOM) at University of\u00a0Hawaii.<\/p>\n<p class=\"p1\">With the school celebrating\u00a0its 50th anniversary this year,\u00a0this research is only one of a\u00a0number of achievements currently\u00a0putting JABSOM in the\u00a0spotlight. For three years, it has\u00a0ranked No. 1 in the National\u00a0Institutes of Health research\u00a0awards for public medical\u00a0schools without a university\u00a0hospital. Just recently, U.S.\u00a0News &amp; World Report ranked\u00a0it No. 19 in the nation as a top\u00a0medical school, up from last\u00a0year\u2019s No. 57 spot.<\/p>\n<p class=\"p1\">\u201cWe\u2019re very excited,\u201d says\u00a0JABSOM dean and University\u00a0of Hawaii Cancer Center interim\u00a0director Dr. Jerris Hedges,\u00a0on the school reaching this 50-year milestone.<\/p>\n<p class=\"p1\">\u201cWe have a lot to be proud\u00a0of.\u201d<\/p>\n<p class=\"p1\"><strong>EBOLA: A DISEASE<\/strong><\/p>\n<p class=\"p1\">Around last September, Dr.\u00a0Axel Lehrer answered questions\u00a0from local reporters in an\u00a0attempt to ease public concern\u00a0over Ebola. It was, he recalls\u00a0telling them, most likely to stay\u00a0within Africa and not affect the\u00a0U.S.<\/p>\n<p class=\"p1\">A few weeks later, the first\u00a0case of Ebola in the U.S. was\u00a0reported.<\/p>\n<p class=\"p1\">Fortunately, it did not reach\u00a0epidemic conditions in the U.S.,\u00a0but the truth is, Lehrer admits,\u00a0you just don\u2019t know when the\u00a0next public health crisis will\u00a0occur.<\/p>\n<p class=\"p1\">He began developing a\u00a0filovirus vaccine for Ebola 12\u00a0years ago while working for\u00a0Hawaii Biotech Inc. Last year,\u00a0he joined the Department of\u00a0Tropical Medicine, Medical\u00a0Microbiology and Pharmacology\u00a0at JABSOM, and in\u00a0May the school announced it\u00a0would partner with Hawaii\u00a0Biotech to continue work on\u00a0the vaccine.<\/p>\n<p class=\"p1\">In 2005, the recombinant\u00a0subunit vaccine reached efficacy\u00a0after successfully protecting\u00a0mice. Though it is important to\u00a0note that there is no infective\u00a0virus in Hawaii and that all testing\u00a0by JABSOM is done by\u00a0collaborators on the Mainland.<\/p>\n<p class=\"p1\">The next step, says Lehrer,\u00a0is clinical testing on humans, a\u00a0process that may take around\u00a012-18 months. It\u2019s easier said\u00a0than done, though, when he\u00a0estimates that it will cost roughly\u00a0$3 million to $5 million to\u00a0get the vaccine into a clinic, and\u00a0another $12 million more to\u00a0complete testing.<\/p>\n<p class=\"p1\">It\u2019s even more difficult\u00a0because funding for the vaccine\u00a0has been difficult at times.<\/p>\n<p class=\"p1\">\u201cBecause I have really been\u00a0working on this for a long time,\u00a0it certainly \u2026 is satisfaction that\u00a0you feel,\u201d says Lehrer. \u201cBut I\u2019m\u00a0also acutely aware of all the\u00a0struggles.<\/p>\n<p class=\"p1\">\u201cI wish that our biggest\u00a0obstacle \u2014 and it is a simple\u00a0one, it is funding \u2014 would be\u00a0relieved because we really think\u00a0that we have a very good solution\u00a0to the problem in our\u00a0hands,\u201d he adds.<\/p>\n<p class=\"p1\">If it tests successfully in\u00a0humans, Lehrer and his team\u00a0will have created a vaccine that\u00a0stands out among others in\u00a0development for a few reasons.<\/p>\n<p class=\"p1\">First, the recombinant subunit\u00a0vaccine may be given to\u00a0anyone in any age range regardless\u00a0of pre-existing health conditions\u00a0like autoimmune diseases\u00a0or HIV. Second, unlike\u00a0vaccines that prevent against\u00a0diseases like hepatitis, Lehrer\u2019s\u00a0vaccine may be used to boost\u00a0itself or other vaccines.<\/p>\n<p class=\"p1\">Finally, the vaccine relies on\u00a0purified recombinant proteins\u00a0produced in insect cells. Photo by Nathalie Walker<\/p><\/div>\n<p class=\"p1\">This is one outcome that last\u00a0month attracted another partnership\u00a0for JABSOM with\u00a0Soligenix Inc. Lehrer, with help\u00a0from Soligenix, is exploring\u00a0the potential for his vaccine to\u00a0not require refrigeration. If it\u00a0proves successful, Lehrer\u2019s\u00a0vaccine would not only prove\u00a0extremely useful in areas it is\u00a0needed most, but could drastically\u00a0affect medical practices\u00a0in general.<\/p>\n<p class=\"p1\">\u201cOur immune memory is\u00a0something that you can easily\u00a0mislead,\u201d adds Lehrer. \u201cSo it is\u00a0very, very important for a vaccine\u00a0to be right on target. That\u2019s\u00a0what it looks like we\u2019re able to\u00a0generate with this.\u201d<\/p>\n<p class=\"p1\"><strong>EBOLA:\u00a0A SOCIOLOGICAL CONCERN<\/strong><\/p>\n<p class=\"p1\">To understand Ebola as it is\u00a0in Africa is to know that all cultures\u00a0are not alike.\u00a0More than 85 percent of\u00a0Africans, for instance, are likely\u00a0to see a traditional healer before\u00a0a medical professional.<\/p>\n<p class=\"p1\">The burial process also is different.\u00a0In Africa, a person who\u00a0has died is believed to have\u00a0achieved a higher state of being.\u00a0It\u2019s tradition, therefore, to wash\u00a0and prepare a body for that transition,\u00a0and to show love by kissing\u00a0and caressing the body.<\/p>\n<p class=\"p1\">And like any metropolis, the\u00a0further away you are, the less\u00a0access you have to things like\u00a0modern communication and\u00a0healthcare.<\/p>\n<p class=\"p1\">So maybe a person hugging\u00a0the body of someone who died\u00a0of Ebola had a cut. Or a healthcare\u00a0worker dressed in protective\u00a0gear arrived to take away\u00a0bodies of loved ones before a\u00a0family could complete burial\u00a0rites (thus instilling mistrust,\u00a0making people less inclined to\u00a0seek professional medical\u00a0help). Or a village is simply further\u00a0away from cities and\u00a0received misinformation, or\u00a0slow information, about the disease.<\/p>\n<p class=\"p1\">All of this was, argues Dr.\u00a0Felix Ikuomola of Nigeria, a\u00a0major impetus for the spread of\u00a0Ebola in rural areas in Africa\u00a0\u2014 places like Sierra Leone,\u00a0Liberia and Guinea.<\/p>\n<p class=\"p1\">\u201cTo really prevent another\u00a0outbreak of Ebola and all the\u00a0communicable diseases, we\u00a0need to bring in culture,\u201d says\u00a0Ikuomola. \u201cWe need to understand\u00a0the socioeconomic factors\u00a0of these people.\u201d<\/p>\n<p class=\"p1\">Ikuomola, already a medical\u00a0doctor, currently is working\u00a0toward a Ph.D. in clinical\u00a0research at JABSOM and will\u00a0publish a book on these very\u00a0societal concerns. Titled The\u00a0Ebola Virus and West Africa:\u00a0Medical and Sociocultural\u00a0Aspects, it is slated for release\u00a0next month.<\/p>\n<p class=\"p1\">\u201cI also have a mind of going\u00a0to Africa to give the government\u00a0leaders and policy leaders\u00a0(ideas of) how we can move\u00a0forward in our cultural modification,\u201d\u00a0he says. \u201cWe can kind\u00a0of be proactive instead of waiting\u00a0for when we have a problem.\u201d<\/p>\n<div id=\"attachment_55499\" style=\"width: 810px\" class=\"wp-caption alignnone\"><a href=\"https:\/\/hawaii.carpentermediagroup.com\/midweekpro\/wp-content\/uploads\/sites\/12\/2015\/06\/screen-shot-2015-06-23-at-12.20.54-pm.png\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-55499\" class=\"size-large wp-image-55499\" src=\"https:\/\/hawaii.carpentermediagroup.com\/midweekpro\/wp-content\/uploads\/sites\/12\/2015\/06\/screen-shot-2015-06-23-at-12.20.54-pm-800x636.png\" alt=\"JABSOM\u2019s first four-year class of M.D.s graduated in 1975. Photo courtesy JABSOM<\/p><\/div>\n<p class=\"p1\"><strong>EBOLA: AN EXAMPLE<\/strong><\/p>\n<p class=\"p2\">When you look at the work\u00a0that Lehrer and Ikuomola have\u00a0accomplished, the result is a\u00a0synergy that encapsulates all\u00a0that JABSOM aspires to instill\u00a0in its students as an institution\u00a0\u2014 that this isn\u2019t merely a place\u00a0for future doctors to learn, but\u00a0one that also fosters growth and\u00a0relationships within the community.<\/p>\n<p class=\"p2\">Because it does not have a\u00a0university hospital, for example,\u00a0JABSOM works with local\u00a0hospitals to train students.\u00a0Through JABSOM and its\u00a0Cancer Center, the school also\u00a0has formed an alliance with The\u00a0Queen\u2019s Medical Center,\u00a0Hawaii Pacific Health and\u00a0Kuakini Medical Center to\u00a0combat cancer.<\/p>\n<p class=\"p2\">And the bridge between\u00a0medicine and culture that\u00a0Ikuomola hopes to encourage\u00a0in Africa already is taking place\u00a0at JABSOM. The Native\u00a0Hawaiian community in particular\u00a0is one JABSOM has\u00a0worked with for more than a\u00a0decade through its Department\u00a0of Native Hawaiian Health. It\u00a0is the only program of its kind\u00a0in the nation that is devoted to\u00a0improving the health of an\u00a0indigenous population.<\/p>\n<p class=\"p2\">\u201cThis department is a leader\u00a0in terms of helping introduce\u00a0not only cultural issues into our\u00a0curriculum, but also connecting\u00a0us with the community and\u00a0grounding us with the community,\u201d\u00a0says Hedges.<\/p>\n<p class=\"p2\">The school, he notes, constantly\u00a0has been evolving.<\/p>\n<p class=\"p2\">In its future, JABSOM is\u00a0looking to expand its graduate\u00a0medical experience and other\u00a0possible fellowship opportunities.\u00a0Work like Lehrer\u2019s also\u00a0makes Hedges hopeful that\u00a0continued research efforts will\u00a0create cutting-edge scientific\u00a0job opportunities within the\u00a0state.<\/p>\n<p class=\"p2\">\u201cA lot of that will depend\u00a0upon doing some unique collaboration\u00a0with commercial\u00a0entities, like that partnership\u00a0that (Lehrer) developed with\u00a0Hawaii Biotech. That\u2019s the sort\u00a0of thing we want to see continue\u00a0to develop,\u201d says Hedges.<\/p>\n<p class=\"p2\">\u201cThis whole complex was\u00a0built on the idea of the school\u00a0evolving beyond its role of educating\u00a0physicians to also have\u00a0a significant impact on the science\u00a0that\u2019s developed in Hawaii\u00a0for Hawaii,\u201d he adds of the\u00a0school\u2019s Kakaako campus that\u00a0opened in 2005. (The school\u00a0initially opened at Leahi\u00a0Hospital.)<\/p>\n<p class=\"p2\">It\u2019s a seemingly lofty goal\u00a0being brought to reality at JABSOM\u00a0that Hedges credits largely\u00a0to the school\u2019s faculty and\u00a0staff \u2014 1,200 of whom are volunteers.\u00a0Ultimately, it is they\u00a0who will pave the way for\u00a0future generations of doctors,\u00a0researchers and scientists, and\u00a0it is a testament to their commitment\u00a0to work for and with\u00a0the community.<\/p>\n<p class=\"p2\">\u201cOften people say, \u2018Oh, what\u00a0are we giving back to the community,\u2019\u201d\u00a0says Lehrer. \u201cYou\u00a0have a reason you want to\u00a0research something and why\u00a0you\u2019re fascinated by it. Us now,\u00a0as faculty here, we are the ones\u00a0to bring that fascination from\u00a0our generation to the next.\u201d<\/p>\n<p class=\"p2\"><em>JABSOM hosts its 50th\u00a0anniversary gala Saturday, July\u00a018, at Sheraton Waikiki.
