The Agency has received a request for a public information meeting on the following certificate of need application: #21-24A for administrative review from 45 Alamo Management Partners, LLC for the establishment of chronic renal dialysis services at
\n4-831 Kuhio Highway, Unit Nos. 400-412, Kapaa, HI, at a capital cost of $7,212,404. The meeting will be held via video-teleconference on:<\/font><\/p>\n

If you need auxiliary aid/service or other accommodation due to disability or limited English proficiency, please contact the State Health Planning and Development Agency at (808) 587-0788 (voice) or (808) 587-0854 (TTY) or (808) 587-0783 (fax) or by email at <\/font>shpda@doh.hawaii.gov<\/u><\/font> as soon as possible, preferably 4 working days prior to the date of the meeting. If the Agency’s Interactive Conference Technology (ITC) connection for the remote meeting is lost, the meeting will be recessed for up to 30-minutes until the connection is restored. In the event the Agency is only able to re-establish an audio connection, the meeting will continue as audio-only. To connect via audio-only, refer to the instructions for Phone Access above.
