Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested person(s) regarding the following. Ms. Naomi Takemoto has applied for a variance for the maximum of five (5) years from Hawaii Administrative Rules, Chapter 11-62.
\n Ms. Sarah Broward, Lox Hawaii, LLC is the authorized agent to act for the applicant. The variance request is for the construction of a new individual wastewater system to be installed at 5134 Haleilio Road, Kapaa, Hawaii 96746 and TMK: (4) 4-1-013: 011.
